2010-2011 Enrollment Paperwork

Below is the list of enrollment/re-enrollment paperwork for the 2010-2011 school year that will need to be printed and returned to our office.  ALL COMPLETED PAPERWORK must be signed and returned to the Carden office on or before July 30, 2010.  Every form in the "Required" section of this list must be turned in by the deadline for your student's enrollment to be considered complete, and to avoid a late fee.  Please be sure to read the list thoroughly as new students and 7th grade students will require proof of immunization to complete enrollment.

REQUIRED FORMS for every student

Student Information Sheet  This comprehensive form must be completed for every student!

Handbook for Students and Parents 2010-2011  Please read the handbook as there have been changes.

Internet Use Release Your student will not be allowed to use the internet during computer class without this release.

Photo Use Release

Your student will not be included in activity photos used for the school's website or marketing materials without this release.  We never use student names on the internet or release student names for publicity.

Family Directory Form

Please print and complete the contact information, etc.,  you would like included in this year's Family Directory.  If you would prefer not you have personal information included in the directory, please return the form indicating that you would prefer not to participate.  (this form is not in a fillable pdf format; print and complete)

ADDITIONAL INFORMATION REQUIRED for NEW and 7th GRADE STUDENTS

Immunization Requirements

Requirements for Kindergarten, all new students, and students starting 7th grade.  Must provide copy of immunization record

Health Examination Form

All first grade children are required to have a health check-up by the physician of your choice or by the public health department.  This check-up must have been completed within 18 months of the student entering first grade.  Students entering first grade must return this form with their registration.  Kindergarten students may also return this form, however, please be aware of the 18 month restriction.

OPTIONAL FORMS

Prescription Medicine Release Form

This is only necessary if your student will need to take medication during school hours, including OTC medication.
